SUMMARY: The Physical Therapy Assistant implements an individual patient therapy care plan that has been prepared by the Physical Therapist and works under the supervision of the Physical Therapist. PRIMARY FUNCTIONS: - Identifies possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, and contra-indicated medication and reports to supervisor. - Observes and records activities and findings in the clinical record and reporting to the physician the client's reaction to treatment and any changes in the client's condition. - Instructs other health team personnel including, the appropriate, home health aide and family members in certain phases of physical therapy with which they may work with the client. - Participates with staff, client, and physician in discharge activities and assists the Physical Therapist in completing the Physical Therapy discharge summary within the designated framework of the Agency policies. - Communicates effectively with all those providing care. - Under the supervision of the physical therapist, assists in evaluation and development of the rehabilitative plan of care and in periodic re-evaluation as indicated. - As d...
