Position Summary: *Must reside in Ohio* This is a full-time field-based position in Ohio. This position requires routine travel up to 75% within the assigned region. The Community Health Worker (CHW) serves the community as a bridge between the member (community) and the healthcare system (providers) through outreach and education. By providing these services, CHWs help Aetna members attain and maintain better health outcomes, improve relationships with healthcare providers and help both parties to become more acculturated with each other.
