JOB DESCRIPTION: SUMMARY OF POSITION Responsible for assuring Care Coordination and Support (CCS) services are provided in compliance with the Pathways to Success Care Coordination and support organization requirements. Works collaboratively with other service teams to meet the needs of clients assigned via Pathways. Responsible for monitoring effectiveness of services as well as assists with community-based child and family team meetings, IMCANS review, crisis prevention and safety planning.
