Job description

Position Summary This role supports a new Psychiatry practice and requires a high degree of flexibility. The schedule is Monday–Friday, 8:00 AM–5:00 PM , with no weekends or holidays . This is an onsite position located at North MOB . Travel to other facilities will be required during the initial 3‑week training period . Top Responsibilities: - Serve as a liaison between physicians, providers, and patients - Provide referral resources and ensure patient needs are met outside of therapy - Support patient registration and onboarding - Deliver patient education and guidance - Manage patient caseloads through in‑person, telephonic, and electronic communication - Conduct outreach calls with patients/caregivers to encourage program participation - Coach patients to help them meet personal and clinical goals - Identify patients needing intensive case management - Assist with scheduling provider visits, arranging transportation, and preparing visit agendas - Educate patients on preventive monitoring and help caregivers understand response plans - Help patients access community and government-based service agencies, including assisting with paperwork - Document all activities and reconcile...
