What You’ll Do: - Provide high-quality bedside care and symptom management for hospice patients. - Educate and support patients and families through individualized care plans. - Collaborate with an interdisciplinary team to coordinate and implement the plan of care. - Utilize technology to enhance care coordination and communication. - Approach patient care holistically, addressing physical, emotional and spiritual needs. - Enjoy flexible scheduling that support work-life balance.
