Schedule Full-Time: 36 Hours/Weekly: 3p-3a, every third weekend, holiday rotation General Summary: Interviews patients and reviews medication-related documentation for the purpose of compiling a medication history for patients being admitted to the Hospital. Compiled history information is entered into the patient's electronic health record for the purpose of facilitating medication reconciliation by the admitting provider. May perform a variety of functions which impact the medication reconciliation process and patient care. Essential Functions: - Direct involvement with patients, care givers or family members upon arrival to obtain and document their home prescription and non-prescription medications. - Effectively communicates across all health care disciplines inpatient and outpatient to ensure all information is obtained and documented. - Collects an accurate best possible medication list of the patient’s home medications in anticipation of hospital admission. - Gathers information for best possible medication list from various resources (patient interviews, physician offices, community pharmacies, historical encounters, extended care facilities, or other acute care facilities...
