Department: Pharmacy Services Shift: Primarily Days (United States of America) Employee Type: Regular Per Diem Type (if applicable): Minimum Pay Range: $67.48 - $84.35 Job Description: POSITION SUMMARY The Refill Authorization and Medication Access Pharmacist provides pharmaceutical support to clinic providers and patients by managing refill authorization requests, select therapeutic alternatives that align with payor formularies, and address medication access related issues, under Collaborative Practice Agreements (CPAs). The pharmacist collaborates closely with providers, nurses, pharmacy technicians, and other healthcare team members to optimize patient access to safe, effective, and cost-conscious medications, while maintaining compliance with organizational, state, and federal regulations. Education/Licensure/Certification: · Current California Board of Pharmacy – Licensed Pharmacist required. · Doctor of Pharmacy (PharmD) required.
