Employment Type: Part time Shift: Description: St Agnes Home Health Part-Time 24 hrs a week on avg or more. 3 days a week. Flexible schedule Job Title: Contingent Speech-Language Pathologist (Home Health) Provides professional speech and audiology diagnostic and therapeutic services to clients as authorized by physicians and in compliance with state practice acts and licensure requirements. Supervision: Serves as preceptor for new staff and provides functional supervision of HHAs. Essential Functions: - Prioritize and plan visits based on client condition, physician orders, and available resources. - Maintain knowledge of laws, regulations, ethics, and agency policies. - Assess clients at admission and required intervals using standardized documentation. - Develop and revise treatment plans with physician and client, including attainable goals. - Make referrals and coordinate care with other disciplines. - Apply professional judgment in evaluations and interventions. - Implement preventive, therapeutic, and rehabilitative procedures per treatment plan. - Educate clients and families on communication and swallowing improvement. - Report changes in client condition promptly and obtai...
