The Physical Therapist is primarily responsible for the implementation of standards of care for physical therapy services and for adherence to all conditions outlined in the Service Agreement. Any direct client care provided will be performed as defined by the state Physical Therapy Practice Act. Position: Physical Therapist Case Manager (PT CM) for Home Health Salary Range: $91,000 - $110,000 annually, depending on experience Service Area: North Central (coverage area: Golden Valley, Robbinsdale, Brooklyn Center, Brooklyn Park, Anoka, Coon Rapids, Oak Grove & surrounding). Plus mileage reimbursement of 72.5 cents per mile.
