POSITION SUMMARY: The Transfer Center Nurse is responsible for coordinating patient admissions and transfers between Boston Medical Center Health System facilities. This role ensures a smooth, efficient, and safe transition of patients by acting as a liaison among patients, healthcare providers, and receiving facilities. The position requires strong clinical knowledge, organizational skills, and the ability to communicate effectively with diverse stakeholders. Manage patient transfer requests, prioritizing based on medical urgency, bed availability, and resource allocation. Serve as the primary point of contact for physicians, nurses, case managers, and sending or receiving facilities throughout the admission and transfer process.
