WelbeHealth PACE helps seniors stay in their homes and communities by providing comprehensive medical care and community-based services. We serve the most vulnerable seniors with better quality and compassion in a value-based model. The Personal Care Assistant is a vital part of our team, providing aid to our participants as well as contributing to the care planning process as a member of our interdisciplinary team (IDT). Reporting to the Day Center Manager, the Personal Care Assistant will provide safe delivery of personal care services to participants at the WelbeHealth day center. This role is different because Personal Care Assistants at WelbeHealth: Schedule: 8-hour shift between 6:30 AM–6:30 PM PST M-F. This role follows a rotating schedule, and all team members must be available to work all three shifts: 6:30 AM–3:00 PM PST, 8:30 AM–5:00 PM PST, and 10:00 AM–6:30 PM PST.
