Job Summary The Community Health Worker goes beyond medicine by connecting AbsoluteCare members with resources that help address their social needs and overcome barriers to care. This role fosters trust and builds meaningful relationships with members, meeting them wherever they are, whether at home, in the community, at the hospital, or in an office setting. The Community Health Worker will also reach out to new members, educating them about AbsoluteCare and guiding them through enrollment in both internal and external services. This position operates in a matrixed environment, with dual reporting to the Community Team Care Manager for daily care management and the Engagement Manager/Director for performance evaluation, strategic alignment, and program oversight.
