Job Purpose: The Case Manager is primarily responsible for assisting in patient care to promote effective education, self-management support, and timely healthcare delivery to achieve optimal quality and financial outcomes. This individual is a vital member of the Case Management Team and services as an advocate for patient healthcare needs – coordinating care to minimize the fragmentation of healthcare delivery systems and thereby enhancing quality, cost-effective outcomes .
