Position Overview: As an Advanced Practitioner at CapitalCare Family Practice Slingerlands, you will provide full-spectrum family medicine care alongside experienced physicians and Advanced Practice Providers. The practice emphasizes preventive medicine, chronic disease management, and strong patient relationships, with modern tools such as telemedicine and a secure patient portal to support coordinated care and patient access.
