Job Summary: We are seeking a compassionate and dedicated Disability Support Worker to provide assistance and support to individuals with disabilities. The successful candidate will be responsible for helping clients with daily living activities, providing emotional support, and promoting independence and community integration. The role requires excellent communication skills, the ability to work independently, and a willingness to travel within the area of service delivery. Responsibilities : Qualifications : - Provide safe, consistent, and trauma-informed care to children and young people in OOHC environments. - Support the implementation of individualised case plans and behaviour support plans as directed by the host agency. - Supervise and support children and young people in all facets of daily routines and activities including but not limited to education or vocation, appointments, family time, recreation, and transport. - Participate in regular team meetings, supervisions and training as directed by the host agency - Accurately document incident reports, daily progress notes, and other records in line with host agency protocols using host agency documentation systems. - Buil...
