Job description

This is a flex nursing role which provides case management, home health and clinic visits in Napa county. Schedule : Weekdays Monday to Friday 8am-4:30pm. No weekends or holidays! As a member of the Interdisciplinary Team, the Registered Nurse (RN) participates in the development of an integrated plan of care and provides direct nursing care to our frail elderly participants. The Flex RN position supports and provides coverage for the PACE RN Case Manager position and PACE clinic nurse and home health nurse positions. This dynamic position utilizes the full-spectrum of high level nursing skills-- from critical -thinking care coordination and triage skills, to hands-on, clinical nursing skills.
