Role Overview: We’re seeking a passionate and experienced Clinician - Restorative Care to play an integral role in delivering clinical oversight and coordination within our Support at Home Program’s Restorative Care Pathway. Reporting to the Head of Care - Fully Coordinated, you’ll provide multidisciplinary clinical leadership across short-term restorative episodes (up to 16 weeks), ensuring care delivery is intensive, purposeful, and aligned with participant goals focused on reablement, independence, and functional improvement.
