Our Company Grane Hospice Care: Overview: Grane Hospice: Float nurse between Harrisburg, York and King of Prussia branches - Can also be asked to assist in other parts of the state. Monday - Friday 8:00am to 5:00pm are the hours. Can be asked to participate in back up on-call rotation; holiday on-call participation one summer holiday, one winter holiday. The Statewide Float Nurse (RN) provides staffing support to hospice branches throughout the state by fulfilling temporary staffing needs, as determined by leadership. In this role, you will support your patients by providing direct care and coordinating with the interdisciplinary team to ensure that both patients and their families have an optimal care experience. Responsibilities: - Travels within the state to provide care to hospice patients. - Completes an initial, comprehensive, and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). - Provides professional nursing care by utilizing all elements of nursing process.
