BASIC PURPOSE: To provide case manager services to young person’s ages 15 - 25 years, with early onset psychosis in the CSC program, which allows them to live successfully in the community and grow their recovery. ESSENTIAL DUTIES & RESPONSIBILITIES: Assist in development of CSC program processes in accordance with grant requirements. Assist in development and maintenance of relationships with external providers to coordinate resources and referrals. Provide assessment, treatment planning, education and assistance in developing and completing activities of daily living, recreational and social skills, providing symptom monitoring / management and education of symptom management tools, as well as crisis intervention. Engage and coordinate with family/support systems. Attend all team and client-related treatment meetings.
