Union Position: No Department Details: Come join the Care Management team! This full time position supports patients at our Southpointe Internal Medicine Clinic. This clinic has 14 providers. We are looking for someone who is self-motivated and can work independently as well as adapt well to changes. Hours are Monday through Friday 8am to 5pm with no weekends or holidays. Bachelor's degree in nursing required and RN experience preferred. Summary: Responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care.
