Using the nursing process, you will assess, develop, implement, and evaluate individualized care plans while proactively anticipating patient needs. Serving as the primary clinical contact, you will provide support, education, and guidance to patients, families, and caregivers, ensuring they feel confident and informed. In this role, you will coordinate multi-disciplinary and multi-site care, facilitate seamless communication among providers, and organize all necessary education and training for the care team.
