Job description

Position Summary: Responsible for direct referral source service delivery, providing information on programs and services to grow existing business and develop new business and coordinating referrals to ensure optimal transitions from facility to home. Reports to: Regional Manager, Transitional Care & Business Development Functions & Responsibilities: - Collaborates with clinical and hospice teams to promote smooth transitions from facility to home and promotes optimal communication between VNA Care and referral sources. - Participates in direct referral source servicing by maintaining key accounts, building new business, identifying needs, problem solving and meeting or exceeding expectations of referral sources. - Educates key targets on the benefits and quality of care for patients using VNA Care and elicits feedback regarding programs and services - Participates in developing a sales and marketing plan for home health, palliative care and hospice services with the goal of enhancing account relationships and increasing referrals. - Develops and implements strategies for building relationships with physicians, referral sources, managed care and assigned accounts. - Assists with t...
