Location: Calmont Operations Building Department: Clinical Support & Prevention Shift: First Shift (United States of America) Standard Weekly Hours: 40 Summary: Provides support for all Service Coordination Department functions which include utilization management, service coordination, and population health management. This position also provides health education and care coordination services for CCHP members telephonically, in home, or health facility settings. Education & Experience: High School graduate minimum. Associate degree preferred but not required. Three years' experience in a managed care or medical office/hospital environment.
