Description What We're Looking For: Are you a proactive problem-solver who takes pride in delivering meaningful work that makes a lasting impact? We're looking for a driven and detail-oriented professional to join our team as a Community Health Worker: State Opioid Response. In this role, you'll play a vital part in helping clients and their families to navigate and access community services, other resources, and adopt healthy behaviors, helping us move forward with purpose and precision. The ideal candidate values continuous learning, leads with a welcoming spirit, takes ownership of their work, and is passionate about supporting people and building stronger communities. Essential Job Duties: - Conduct required monthly home visits to provide clients with learning opportunities, guidance, and resource connections to support their recovery. - Complete and maintain accurate written and electronic database records in accordance with ODH and/or grant funder standards. - Coordinate and/or conduct initial and ongoing screening/assessments of clients; determine concerns, priorities, and resources for each client and assist them with creating goals.
