Company: Providence at Home with Compassus Territory Coverage (candidate must be local and willing to travel to below cities): La Habra, Fullerton, La Mirada, Cerritos, Buena Park, La Palma and surrounding areas Schedule : 5 weekdays per week, with rotating weekends 8-hour shift/ 40-hour work week Position Summary: The LPN/LVN provides holistic nursing care to patients and families under the supervision of a Registered Nurse, following the Plan of Care. This role supports patients and families by anticipating changes in condition, promoting patient and family choice, ensuring continuity of care, and enhancing quality of life for patients at the end of life.
