Position Overview: The Clozapine Coordinator supports the delivery of the Clozapine Clinical Care Coordination service across the South West region, assisting General Practitioners (GPs) and authorised medical providers in the management and monitoring of people receiving Clozapine pharmacotherapy. The role provides clinical care coordination, mental and physical health monitoring, medication support and service navigation for individuals receiving Clozapine treatment, particularly during transition from inpatient care to community-based support.
