What You'll Do: - Maintains prompt response time to patient/caregiver phone calls to ensure 24 hour access to service - Ensures prompt access to hospice care by managing all available resources when office is closed - Provides appropriate intervention, e.g. emotional support, education, nurse visit, in response to stated problem - Triages visits to provide care to patients/families in crisis - Ensures continuity of established plan of care for patients and families receiving hospice service - Maintains confidentiality, discusses patient information discreetly with caregivers and necessary staff members - Assesses the need for patient and family education, provides education as needed and documents in medical record - Obtains appropriate physician orders to achieve symptom management - Directs others i...
