JOB PURPOSE: The Case Manager will provide comprehensive case management to individuals and/or families experiencing homelessness and/or housing instability by utilizing a traumainformed care and a strengths-based approach with the goal of increasing their quality of life and maintaining the highest possible ability to function within the community. QualificationsESSENTIAL JOB RESPONSIBILITIES: Develop effective, trusting relationships with program participants by utilizing a trauma informed care and strength-based approach. Model positive interactions, verbal guidance, and effective communication. Utilizing the Housing First Model to advocate for and facilitate participant's access to community resources; housing resources, school programs, vocational opportunities or services, recreational activities and relevant mental health services and benefits; assist participants to develop natural resources and contact social support networks.
