Department: Home Care: Shift/schedule: Full Time (40 hrs/wk): GENERAL SUMMARY: Works under the supervision of the Nurse Manager or Asst Nurse Manager. The RN Admission Nurse evaluates and admits patients for all home care disciplines and makes regularly scheduled visits as needed. Duties include: admission; revisit; OASIS data collection; developing the plan of care; referral of ancillary services e.g. rehabilitation therapists, social worker and home health aide and completion of required documentation. Assists with QAPI programs. Performs other duties as assigned.
