Department NWV HOME HEALTH - 517151: Worker Sub Type Regular Work Shift Pay Grade: 183 Job Description The Home Health Physical Therapist provides direct patient care in the patient's home environment as part of a multidisciplinary team including nursing, physical therapy, occupational therapy, speech, social work and home health techs. The Physical Therapist evaluates patients and use professional judgment to design the individualized plan of care which best meets the patient's needs. The Physical Therapist provides direct, one-on-one patient care, and will also supervise care provided by assistants. The Home Health agency relies heavily on technology; email is a primary means of communication, and the therapist will use a laptop to complete documentation in an electronic record. The patient population varies widely across the spectrums of acuity, age, and diagnosis. The Physical Therapist works under the direction of a clinical team manager with support from a rehab clinical coordinator.
