As we continue to expand our clinical programs to meet increasing community needs, we are seeking RN Case Managers to join our teams in Onondaga, Oneida, Oswego, Jefferson, or Cayuga Counties! We believe in listening to our nurses, supporting them, and setting them up for success every day. Home visits are scheduled Monday–Friday, 8:00 AM–4:30 PM, with no on-call requirement, and a one weekend a month rotation.
