Scheduled Hours 40 Position Summary: Responsible for assessing, planning, coordinating and evaluating the care of patients. Job Description: Primary Duties & Responsibilities: Conduct a comprehensive bio-psychosocial culturally-sensitive assessment and behavioral health assessments with the client. Synthesize program goals and assessed needs to create an individualized plan of care for each client. Arrange and coordinate service referrals for the client as needed; provide ongoing monitoring of the activities implemented to evaluate progress on the plan of care. Refer clients to public benefits programs as eligible. Independently manage caseload by providing regular contact with enrolled clients and documenting all contacts and service coordination activities in client records as required by program guidelines. Educate clients about the basic medical and psychosocial aspects of HIV/AIDS in order to optimize adherence to care. Adhere to all applicable state laws and the department’s rules and regulations regarding HIV/AIDS programs.
