Position Summary: Provides patient care that optimizes the use of medication and promotes health, wellness, and disease prevention. Essential Duties and Responsibilities include the following: All processes of care/Pharmacist Referrals: Disease Management for Diabetes, COPD, CHF - Regimen review, ensuring safe medication administration, monitoring for desired therapeutic outcomes, identifying and avoiding medication errors and adverse drug events, etc. - Provides pharmacological information by answering questions and requests of health care professionals - Works collaboratively with Management staff in the medication management of patients transitioning from inpatient to outpatient or next level of care - Counseling patients on drug therapies - Reduction of hospitalizations and ED visits, avoid poly-pharmacy issues, drug interactions, assistance w/medication - Provides clinical and educational support to staff regarding pharmacy and medication issues - Knowledge of chronic disease/conditions and provide treatment goals - To provide disease specific education to patients and improve patient participation in care - To educate health care workers and the community about the importance...
