POSITION SUMMARY: The substance use case manager will provide comprehensive case management services to individuals with substance use disorders. This role will focus on assessing client needs, developing individualized service plans, coordinating treatment, and support services. This will help promote long-term recovery. The case manager will work collaboratively with the senior counselor, client, families, treatment providers, and community resources to ensure continuity of care and support harm reduction, recovery oriented goals. This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to teamwork and cooperation. QUALIFICATIONS: EDUCATION: Bachelor's degree in related field required. LICENSE/CERTIFICATION: CADC preferred. . Valid Driver's License required. Annual, American Heart Association BLS and Heart Saver First Aid required within 90 days of hire or transfer. CPI required within 90 days of hire or transfer.
