STATUS: Non-exempt, full time: Monday-Friday (8:30am to 4:30pm): No on-call responsibility: HOURLY RATE: up to $24.00 per hour SUMMARY: The Healthcare Coordinator provides one-on-one health coordination and support to older adults residing within a community-based setting within designated surrounding counties. The Healthcare Coordinator works closely with the RN Program Coordinator assisting clients with learning how to manage their healthcare as directed by medical providers, with the goal of working towards independence when possible. Healthcare coordination services would be provided to a case load of approximately 25 to 30 clients, depending on complexity of needs. This role does not involve hands on, clinical nursing.
