At NorthBay Health the Physical Therapist works under the supervision of the Rehabilitation Supervisor and Rehabilitation Services Manager. The staff physical therapist, in accordance with his/her evaluation findings and the physician’s referral, plans and implements specific treatment programs for individual patients according to the principles and practices of physical therapy. Collaborates with the interdisciplinary treatment team. PRIMARY JOB DUTIES: Provides physical therapy services in inpatient and outpatient settings during assigned shift during department operating hours, including assignment in rotation of weekend and holiday coverage. With physician’s referral, evaluates patients, develops specific treatment plans and goals, administers procedures and modalities specific to the learning needs and age of the patient, and plans for discharge utilizing appropriate Physical Therapy techniques. Acts as a Rehab case manager for assigned patients. Prepares and maintains accurate and current patient records in accordance with departmental policies and procedures.
