Department: 38494 Enterprise Pharmacy - Pharmacy: Population Health Status: Full time Benefits Eligible: Yes Hou rs Per Week: 40 Schedule Details/Additional Information: The Population Health Virtual Pharmacy Team performs a variety of programs and services using population health management strategies and virtual communication to care for patients across the Advocate Health Midwest Region. These include chronic disease management using collaborative practice agreements to adjust and monitor medications, medication therapy management referrals, medication adherence outreach to patients with chronic diseases, and opioid weaning support. It is an ever-evolving environment, where we engage patients in self-management of their chronic diseases and collaborate with other healthcare professionals to improve health outcomes. The Population Health Clinical Pharmacist – Heart Failure will focus on heart failure patients for post-discharge follow-up via telephone and/or video visits. The pharmacist would be joining a team of pharmacists and pharmacy technicians with varying pharmacy backgrounds. Hours will be 8am to 5pm, Monday through Friday.
