Job description

Description IPS Case Manager will provide case management services to IPS youth and their families. The IPS Case Manager will be responsible for conducting assessments, monitoring safety in home and program, monitoring school progress & attendance and providing referral and linkages. He/She will provide on-site programming including, but not limited to, academic workshops, tutoring, recreation, field trips, etc. IPS Case Manager will report to the site IPS Counselor and IPS Supervisor. Requirements: - Provide Case Management to youth and families of the IPS program, including monitoring youth in home & on site, provide referral & linkage and participate in crisis intervention - Conduct mandatory home visits when required, with the assigned Counselor - Conduct and or participate in assessments of youth with parents/guardian input. This includes intake, Service Plans, Discharge Plans etc. - Collect information about the family through use of interview, case history, and observational techniques - Link parents to support services related to Housing or Social Services - Participate in monthly case review meetings with the team and DHS liaison to discuss youth progress - Document all se...
