Position Purpose: The Population Health Program Manager reports to the Director of Practice Operations and leads the development, implementation, and performance management of population health initiatives aligned with organizational strategy. This role collaborates with clinical, operational, and analytics leaders to drive care transformation, support physician engagement, and implement programs that improve quality, outcomes, and value across the continuum of care.
