Reablement Assistant Practitioner | 9553 | permanent contract | £31,537 to £33,699 per annum (Scale G) | 37 hours per week | The Hive, Norwich Community Hospital, Norwich The post is currently Monday to Friday. Weekend working may be reintroduced in the future on a rota basis or as needed by the service. Would you like to work in the community as part of our well-trained team of Occupational Therapists and support workers, creating person-centred personal plans helping people set and achieve their goals following an illness? Then this role is for you. We have opportunities available to visit people in their own homes on the north side of Norwich to assess their social care needs. You may also work across other areas of Norwich depending on the needs of the service. Norfolk First Support delivers services to adults across Norfolk in their own homes for up to 6 weeks, assisting them to regain skills they may have lost following an illness or hospital admission, enabling them to regain their independence as much as possible while also reviewing any ongoing support needed.
