The Registered Nurse Case Manager supports hospice patients receiving care in their homes or facility settings by providing skilled nursing care and coordinated, compassionate end-of-life support in partnership with the interdisciplinary team. Schedule, Hours & Territory: - Standard operating hours are Monday through Friday, 8:30 AM to 4:30 PM - Territory: Michigan - Alma, Bay City, Midland, Mt.
