Terms and Conditions apply** The Nurse Case Manager demonstrates a high level of expertise in the assessment, diagnosis, and treatment of the complex responses of individuals, families, or communities to actual or potential health problems; prevention of illness and injury; maintenance of wellness; and provision of comfort. Licensure, Certification, or Registration Requirements for Hire: Current RN licensure in Virginia or eligible or compact state Licensure, Certification, or Registration Requirements for continued employment: Current RN licensure in Virginia AHA BLS HCP Certification or equivalent Experience REQUIRED: Three (3) years of experience including two (2) in a specialty area Experience PREFERRED: Two (2) years case management/care management experience Education/training REQUIRED: Baccalaureate Degree in Nursing from an accredited School of Nursing Education/training PREFERRED: Certification in specialty area as appropriate Independent action(s) required: Makes appropriate decisions and initiates appropriate interventions in the provisions of care coordination within the boundaries of professional practice,...
