Registered Nurse (RN) Case Manager – Staten Island Per Diem | Field-Based | Concierge Home Care Affirmed Home Care, New York’s premier private-pay home health agency, is seeking a compassionate and experienced Per Diem RN Case Manager to support clients throughout Staten Island, NY . This field-based role is ideal for an independent nurse who values flexibility, autonomy, and delivering high-quality, personalized care in a home setting.
