ESSENTIAL DUTIES AND RESPONSIBILITIES: Educate and refer clients to community agencies and businesses that appropriately address the clients' needs for food, clothing, shelter, employment transportation, etc. Utilize and manage Electronic Health Record (EHR) systems for data entry and client information tracking, ensuring program alignment. Assist clients with establishing or re-establishing support systems within their community. Act as advocate on behalf of the client to improve access to the services needed. Participate in clinical staffing decisions regarding the client’s current strengths and resources and their need for additional resources.
