Description At Axiom Care, we pride ourselves on offering a real path to recovery from addiction to underserved populations such as those with low incomes and the prison re-entry population. We achieve this by offering multiple levels of care along with medical services, evidence-based professional counseling, case management, housing, employment assistance, and more throughout the Phoenix metro area. Duties/Responsibilities: - Provide individual, Family and group substance use counseling with a trauma informed care approach. - Coordination of care with stakeholders, funding sources and natural supports. - Provide culturally appropriate modalities in support of client’s cultural preferences. - Develop, implement, and review clinical documentation of all services provided through client records in the Electronic Medical Record (i.e., assessments, ASAM Continuum, individualized service plans, screening tools, safety plans, progress notes, discharge summary, etc.) for assigned caseload. - Oversees the accuracy and completion of each assigned client’s record in Electronic Medical Record. - Collaborates with each assigned client on individualized service plan goals and Adult Recovery Te...
