Work where you matter. A Brief Overview: Serves a core member of the collaborative care team. Responsible to plan, organize and implement direct service to patients and families. The Behavioral Health Case Worker is responsible for supporting and coordinating the mental and physical health care of patients on their assigned case load. Maintains contact with referring agencies and related people, and with community resources used for referral. Assists patients and families in assessing financial resources. Works closely with care team and community stakeholders to provide best patient care and promotion of Avera.
