Job DetailsJob Location: Port Gamble S'Klallam Tribe - Kingston, WA 98346Position Type: Full Time - 30 - 40 hrs/weekEducation Level: High School/GED - RequiredSalary Range: $20.95 - $24.09 HourlyTravel Percentage: Road WarriorThe Community Health Representative (CHR) directly impacts individuals, families, and the community by working with the CHR Supervisor, Community Health Coordinator, and medical care team to educate, promote, and support healthy lifestyles. This position maximizes the effective utilization of available health care, health education, disease prevention, dental, mental health, and social services. The position operates under the direct supervision of the CHR Supervisor and the guidance of the Community Health Coordinator. Educating: Assists individuals and families in using health facilities, resources, and preventative and curative practices and services in a timely, effective, and appropriate manner. Informs community members of the services available at the S’Klallam Health Center and other resources in our area and provides appropriate informative materials.
