Position Title: Social Worker – MSW / LSW Location: North Hospital, Carmel, IN: Shift: Full-Time, Days: Position Overview: The Social Worker provides face-to-face psychosocial assessments, counseling, and resource coordination to support patients and families through their healthcare experience. This role involves collaborating with the multidisciplinary team to ensure that patients receive safe, timely, and comprehensive care. The Social Worker advocates for patient needs, assists with discharge planning, and serves as a liaison for child and adult protective services, guardianship, foster care, and community resources. The position requires expertise in crisis intervention, legal mandates, and culturally competent care. Key Responsibilities: Meet with patients and families to assess psychosocial needs and provide counseling.
