Hours: PRN (as needed), 8 am – 4:30 pm. Job duties include but are not limited to: Assist patient in the development of a plan of care for ongoing and maintenance improvement of health care. Work to resolve clinical, psychosocial and financial barriers on the patient’s behalf. Responsible for performing utilization review and discharge planning in accordance with all mandated regulations. Perform early intervention during patient stay regarding the disposition of patient. Participate in multidisciplinary patient rounds as indicated. Assure appropriate allocation of facility resources through monitoring and reporting on utilization of services.
