Support staff as needed. JOB DUTIES: · Interview and assess participants to determine eligibility, needs, and appropriate services. · Develop and maintain case files, ensuring all documentation supports service authorization and compliance. · Create and manage Individual Self-Sufficiency Plans (ISPs) in collaboration with clients, aligning goals with program requirements. · Conduct regular case reviews and update participant records to ensure continued eligibility and progress. · Provide outreach, advocacy, and support services to help participants access community resources and remove barriers to success. · Plan and facilitate training opportunities, job fairs, and skill-building events that enhance employment and self-sufficiency. · M...
